Collection of Personal Information

During a visit to the Dynamic Health Physiotherapy Website, it will be clear when DHP will collect personal information. Through form submissions, online chats, and e-mail contact forms, Dynamic Health Physiotherapy may request information such as, but not limited to; name, e-mail address, phone number, and other insightful information to assist with informing our Physiotherapists on a potential patient’s physical status in regards to their concerning issue. At no point will Dynamic Health Physiotherapy request Medical Records, Social Insurance, or Credit/Credit Card information via our website. We request that all users keep mindful not to submit this sort of sensitive data through the DHP Website. Should information be required, you will be directed to our secure site for patient profiles (JaneApp.com) or a DHP Patient Coordinator will be in contact to enter the information manually, either over the telephone or in person at one of our clinic locations. During correspondence via our websites “Live Chat” feature, a Patient Coordinator or Physiotherapist may request information on a potential patient’s condition. Any information given, apart from name and contact details, through our Live Chat function will not be shared in any form.

Anti-Spam

Dynamic Health Physiotherapy employs security and practices to protect our Website visitors and users who submit personal contact information. DHP does not in any circumstances share or sell its e-mail lists to third parties. Where consent is required, DHP ensures that correspondence recipients have expressly opted-in or have given their implied consent to receive communications from DHP. Each email that you receive from us has simple instructions on how to unsubscribe from our email list.

Use of Cookies

IP Addresses and Server Logs

Dynamic Health Physiotherapy deploys the use of social media tracking Pixels (A tracking pixel is an HTML code snippet which is loaded when a user visits a website or opens an email. It is useful for tracking user behaviour and conversions. With a tracking pixel, web developers can acquire data for online marketing and web analysis) on our Website. This assists DHP in understanding where our Website visitors originate from (Facebook, Instagram, Google, and Ads) so that we may determine the effectiveness of our social media advertising campaigns. Dynamic Health Physiotherapy will not sell, share, or otherwise distribute personal information in any way except as provided in this Policy. Dynamic Health Physiotherapy will disclose personal information only when required by law, court order, or governmental regulation, or if such disclosure is necessary in support of any criminal investigation, or other legal investigation or proceeding.

E-Mail and Correspondence 

Dynamic Health Physiotherapy uses third-party providers to send emails, newsletters and updates to individuals who have elected to receive these communications, be it informal communications such as marketing, or formal patient correspondence. Subscriber lists are stored on a secure server via the service provider ActiveCampaign. This third party service provides DHP with statistics on email campaigns, such as the rate of opening, links clicks, and unsubscribe rates. These analytics are used by DHP to monitor trends and improve marketing. Patient correspondence between our patients and our Patient Coordinators are also conducted through Jane clinical management software. Jane Software Inc (302 – 111 Forester Street, North Vancouver British Columbia, Canada V7H 0A6) retains and manages patient data in regards to files, correspondence, contact information, and booking information. The Jane Software Privacy Policy reviews how data is handled on behalf of DHP.

This Terms of Use Agreement is a legal agreement between Dynamic Health Physiotherapy and its website visitors. You agree and hereby submit to the exclusive and preferential jurisdiction of the courts of the province of New Brunswick with respect to all matters relating to your access and use of the Website and the Content as well as any dispute that may arise therefrom and that the applicable law shall be the law of the province of New Brunswick and of Canada.

Waiver

Severability

Termination

Dynamic Health Physiotherapy may, in its sole discretion, cancel or terminate your right to use the DHP Website, or any part of the DHP Website, at any time without notice.
